The first thing one should know about so-called "comfort" saddles (those intended to take the pressure off of men's/women's sensitive areas) is that they require different setup adjustments than a standard saddle. The forward/backward, tilt and height adjustmemts are all critical to getting a seat like this to work and end up quite different than the settings you'd use with a standard saddle.When you have the adjustments "correct", the seat acts more as a support platform than a resting platform. In other words, it's not meant to be just a flat seat on which you rest most of your body weight.
For this seat, the most critical adjustment is the tilt. One reviewer and one commenter stated that the seat's horns dug into the underside of their thighs. That occurs when you don't have the seat tilted down enough which is stated right on the packaging. I have my seat tilted down about 25 degrees. That's a pretty good tilt and coincides with the concept of the seat supporting your riding position but not supporting your entire body weight.
Aside from the steep tilt, my other adjustments including attaching the seat as far back as possible and lowering the seat post a bit from "standard saddle" height. The ISM saddle, like other "comfort" saddles pushes you higher and further forward than is "workable".
The downside of the riding position based on these settings is that, inevitably, more pressure is placed on your wrists. It makes sense because less pressure is placed on your butt. I'm finding that I get a lot of hand numbness toward the end of my ride.
The quality of the seat itself is superb. It has a leather exterior with a firm yet cushioned level of support. It literally looks like it would last 15 years.
With the seat adjusted correctly, I find the seat to very comfortable with, as I mentioned, a firm level of support. The feel is quite different than a standard saddle and that feel translates into zero discomfort in your sensitive areas when your ride is done.
Common criticisms of "comfort" saddles are that you slip off them and that they offer less bike control. I don't slip off this seat at all (it's leather). Nothing beats the control of a saddle horn but the ISM saddle gives me enough control whereby I can still stand straight up on the pedals, squeeze the seat between my thighs and ride down a curving hill with no hands. Hey, just because I ride like a mad man doesn't mean you should take my review less seriously!
It took me less than one ride to get used to the seat and its new feel. Now, I actually look forward to riding on it. Any discomfort I had with my prior saddle is now gone. If it wasn't for the hand numbness, it would be the ideal saddle for me.

After almost three months of use I have to state that this seat is probably as perfect as one could find in a bike saddle. My hand numbness has gone away even though I've actually RAISED the seat for a little more "leg room" - which, of course, should make the numbness worse. As I stated above, there is absolutely zero contact with sensitive areas on your body. You're actually sitting on the seat with your butt (what a concept!) and even your butt's sensitive areas do not come in contact with the seat due to the center cut-out. Buy this seat and your quest for biking comfort is over - period. I can't recommend this seat enough.
